Funding Request Form - For funding that is not part of the current PTO Budget.  Please complete and return request to a PTO Board Member (via email) or the PTO Mailbox. Please note that funding requests need to be approved at a PTO meeting.

Funding Reimbursement Form - To request reimbursement for funds that are allocated in the PTO budget. Return completed form with copies of receipts in the PTO mailbox attn: Sharon McFarland. If you need money right away, contact Sharon to let her know the form is ready.

For direct payment to a business contact Sharon directly to arrange payment.
